We need a fully functional component that duplicates the functionality and data of the IRS w-9 form: https://www.irs.gov/pub/irs-pdf/fw9.pdf

We prefer the form to look similar to the paper form while in full screen on desktop but also useable on mobile devices.

We are willing to pay up to $100.
